asuperioresthetichasbeen in all-ceramic(AC)crowns’ corner.Yettherisingcostof
preciousmetalsandthedevelopmentofnovelceramicmaterialsare
nowchallengingthestatusquo.
Tobetter understand that new reality, theCanadianAgency forDrugs and
Technologies inHealth (CADTH) recentlypublishedaRapidResponseReport titled
Porcelain-Fused-to-MetalCrownsversusAll-CeramicCrowns: AReviewof theClinical
andCost-Effectiveness.

The reviewanalyzes andcompares the5-year survival rates
of several typesof PFMandACcrowns according to their position in themouth.
Theoverall 5-year survival rateof PFMcrowns (95.7% to97.6%)was found tobe
slightlyhigher than that of ACcrowns (90.7% to96.6%).
ACcrownsmadeof newermaterials—
leucite
or
lithiumdisilicate, alumina
(denselysinteredandglass-infiltrated), denselysinteredzirconia
—showed
survival rates similar toPFMcrowns.
Thepositionof thecrowns in themouthdidnot significantly impact the
survival ratesof
PFMcrowns
or
ACcrownsmadeofalumina (densely
sinteredandglass-infiltrated)
or
leucite
or
lithiumdisilicate
.
Findingswere found tobe inconsistentwith regard to
theeffect of theposition in themouthof
zirconia-
basedcrowns
.
Crownsmadeof
feldspathic
or
silicaglass
ceramic
hada significantly lower survival rate
thanPFMcrowns. Their survival ratewas also
lower in theposterior region than in the
anterior one, and the recommendationwas
touse themonly inanterior regionswith
low functional load.
