Step 1:

To make this process go as smoothly as possible, you must have your license number and be a member of the provincial/territorial dental association, or in Quebec a member of the CDA. Consult the Directory of Provincial Regulatory Authorities & Provincial Associations

Step 2:

You must complete a CDAnet subscription form for each dental office that you will be transmitting claims from.

• Download the CDAnet Subscription Agreement at Subscribe to CDAnet.
• Read the CDAnet Subscription Agreement and complete the CDAnet Subscription Agreement form on page 4.
• Check to make sure that you have completed all the mandatory fields on the CDAnet Subscription Agreement form, including provincial license information.
• Do not submit the CDAnet Subscription Agreement form until the license and membership information is confirmed. Important - CDAnet will only process forms that are complete.
• Sign the completed form to confirm agreement to all the terms and conditions regarding the use of CDAnet.
• Fax the completed CDAnet Subscription Agreement form to 613-523-7070.
• CDAnet will contact the office to provide a CDAnet unique identification number (UIN), a CDAnet office number (if not already assigned) and start date.
